  This is in regards to your letter dated 2/9/2016, sent to the above listed patient name.
  I am slightly confused regarding your letter of Not making any recent scheduled appointments as indicated in your letter, and the reason I would not be scheduling appointments is due to Not having health care coverage since the month of October 1, 2015.
  I as well have Not received ANY confirmation, reminder of these appointments, no reminder calls, nor notices or I would contact office to clarify your mistake.
  The last time I had called the office was Only in regards to my daughter, as well a patient of this practice, to confirm the office cancellation of her appointment with which she had received a letter as well for a follow up, and when she had called the office they were unaware of what this appointment was for. I called to get information to confirm if my daughter did have her yearly physical and was informed that it looked as if she had something possibly - a physical.
  Be advised if there are appointments being made for Patient Pamela Plank, and/or T. Plank, you will need to call contact numbers listed on file at Leroy Medical in order for patients to confirm this appointment, and be certain if this was scheduled by patient Only.
  I have No health Care coverage I had called the office months ago regarding this, and informed I cannot make any appointments until I have health care, if deciding.
  If need to discharge patient due to not having insurance coverage, then that's something you- this office will need to determine.
  I will not need any future appointments unless I have medical coverage, as well having to perhaps look into other medical offices, for new patient. My phone is not on at this time, secondary contact number- Taylor PA Plank, Gus Torres- Parent/Dad- number on file of any concerns and/or mishaps this office may have. Neighbor/Patient Janet Pike.
